Cytopathology is the use of specialised diagnostic procedures to study individual cells removed from tissues in order to identify the source and kind of a disease.

During regular diagnostic procedures including bronchoscopy and cystoscopy, cell samples may be taken. To collect cells from particular areas for diagnosis, certain procedures can be employed, such as the Papanicolaou test, also referred to as the Pap test or fine-needle aspiration.

It takes specialised knowledge to correctly identify cells that could endanger a patient, such as precancerous, cancerous, or infectious cells, even though cell sample analysis is frequently a quick process. A qualified cytopathologist, a medical professional who has completed further training to accurately analyse and interpret the data from cytopathology examinations, is responsible for doing this task.

The Pap test was the first test created in the discipline of cytopathology. It is still a crucial component of contemporary cytopathology and is largely regarded as the most successful screening test in medical history.

Since then, the clinical diagnosis of malignant and precancerous disease states, benign tumours, and some infectious diseases has relied heavily on cytopathology. The field now includes numerous approaches for numerous tissues.

The Pap test is a diagnostic procedure that uses a sample of cervix-derived cells. A Pap test can be performed using either a manual liquid test or an automated one.

Cells are placed on a glass slide, fixed, and stained using a combination of dyes during a standard Pap test. Light microscopy is used to examine the cells for anomalies such morphological or nuclear feature changes.

 

For analysis in the more recent automated liquid Pap test, cell samples are put in a vial of preservative. When compared to a conventional Pap test, this screening procedure has shown to be more effective at diagnosing the problem.

Biopsies using fine-needles

The fine-needle aspiration biopsy is the cytopathologist's other main diagnostic technique. A little needle is used in this method to take a sample of cells from just below the patient's skin.

These cells can be preserved, stained, and studied under a microscope similarly to the Pap test. The best diagnostic approach when examining a probable malignant or pre-cancerous lesion is frequently fine-needle aspiration, which enables the sample of a wide variety of tissues with little discomfort to the patient.

 

The excessively high danger connected to the use of fine-needle aspiration has, however, drawn criticism. A 2004 study published in the British Medical Journal estimated that 0.4–5.1% of procedures used to look into liver lesions resulted in needle tract metastases, or the spread of cancer cells along the path of the needle. As well as cases of breast, prostate, and lung cancer, needle tract metastases have been reported.

Despite this, the technique is still a crucial diagnostic tool, and doctors frequently turn to it first when looking into a potential cancer diagnosis.
